Download, print, and complete an expenses form. Please preferably scan copies of your expenses form and supporting receipts, and email to expenses@hayestownchapel.org.uk or please put the hardcopy in the offering box at the back of the main hall, or give to our Treasurer, James Doggrell.

If sending electronically, please retain the original document and receipts until you have received payment in case something hasn't scanned correctly and there are queries.

We encourage everyone who pays sufficient tax to complete a Gift Aid form so that the church can recover an additional 25% on your gift. Preferably please scan copies of your completed Gift Aid form and email this to treasurer@hayestownchapel.org.uk or place the hardcopy in the offering box at the back of the main hall.
